SEAMAN'S RECORD BOOKS

Seamen's record books are issued by the Bahamas Maritime Authority to any seafarer employed on Bahamian registered ships.

Application for Seaman's Record Book Instructions

To apply for a Seaman's Record Book:

1. Download the form  (pdf format) .   Complete & sign the form.

2. Send it to: The Bahamas Maritime Authority, 120 Old Broad Strret, LONDON  EC2N 1AR UK

3. Enclose two recent passport-sized photographs (3.5cm x 4.5 cm)

4. Enclose a fee of £35 or US$50

(Cheques and bank drafts should be made payable to "The Bahamas Maritime Authority". Do not send cash.

The record books are normally dispatched by post.
