The Bahamas Maritime Authority is responsible for the conduct of investigations into incidents involving Bahamian registered ships, including:

Shipping Casualties, injuries and deaths, by natural or other causes, to crew members or passengers

In the event of a major casualty occurring Masters, Owners and Shipmanagers are asked to report the occurrence as soon as possible by telephone to:

A Casualty Report Form [CRF1] and/or a Return of Death. [RBD1] should be completed and returned to the Investigations Department of the Bahamas Maritime Authority, London Office whenever a casualty or death has occurred.

In the case of a natural death of a crew member or passenger the CRF1 will not be required. A Return of Birth [RBD2] should be completed for a birth.
